AIM OF THE STUDY : The aim of thisstudy was to compare geometric indices of hip bonestrength in overweight and control elderly men.METHODS & RESULTS : This study included 16 overweight(Body mass index (BMI) > 25 kg/m2) elderlymen (aged 65-84 years) and 38 age-matched controls(BMI < 25 kg/m2). Body composition and bone mineraldensity (BMD) were assessed by dual-energy X-rayabsorptiometry (DXA). To evaluate hip bone geometry,DXA scans were analyzed at the femoral neck, theintertrochanteric region, and the femoral shaft bythe Hip Structure Analysis (HSA) program. Cross sectionalarea (CSA), an index of axial compressionstrength, section modulus (Z), an index of bendingstrength, cross sectional moment of inertia (CSMI),an index of structural rigidity, cortical thickness (CT)and buckling ratio (BR) were measured from bonemass profiles. Lean mass, body weight, fat mass andBMI were higher in overweight men compared to controls(p < 0.001). CSA and Z were higher in overweightsubjects compared to controls (p < 0.05) at the threeregions (femoral neck, intertrochanteric and femoralshaft). After adjustment for age, CSA and Z of the intertrochantericregion and the femoral shaft remainedsignificantly higher in overweight men compared tocontrols (p < 0.05). After adjustment for either bodyweight, BMI or lean mass, there were no differencesbetween the two groups (overweight and controls)regarding the HSA variables (CSA, CSMI, Z, CT andBR) of the three regions.CONCLUSION : This study suggests that overweightelderly men have greater indices of bone axial andbending strength in comparison to controls at theintertrochanteric and the femoral shaft.
OBJECTIF DE L’ÉTUDE : Le but de cetteétude est de comparer les indices géométriques de résistanceosseuse au niveau de la hanche chez des hommesâgés en surcharge pondérale et normo-pondérés.MÉTHODES & RÉSULTATS : 54 hommes âgés de 65 à84 ans divisés en deux groupes en fonction de l’indice demasse corporelle (IMC) ont participé à cette étude. Legroupe SUR (surcharge pondérale, IMC > 25 kg/m2)comprenait 16 hommes, et le groupe NOR (normopondéré,IMC < 25 kg/m2) 38. Les deux groupes étaientappariés pour l’âge. La composition corporelle et ladensité minérale osseuse (DMO) ont été mesurées parabsorptiométrie biphotonique à rayons X (DXA). Afind’évaluer la géométrie osseuse, les scans au niveau ducol fémoral, de la région intertrochantérienne et de ladiaphyse fémorale ont été analysés par le logiciel HipStructure Analysis (HSA). La surface (CSA) et le momentd’inertie (CSMI) de la surface transversale, lemodule de section (Z), l’épaisseur corticale (CT) et lebuckling ratio (BR) ont été mesurés par le logiciel HSA.La masse maigre, le poids, la masse grasse et l’IMCétaient supérieurs chez les hommes en surchargepondérale (p < 0,001). La CSA et le Z étaient significativementsupérieurs (p < 0,05) chez le groupe SUR auniveau des trois régions (col fémoral, région intertrochantérienneet diaphyse fémorale). Après ajustementpour l’âge, la CSA et le Z restaient significativementsupérieurs dans le groupe SUR au niveau de la régionintertrochantérienne et de la diaphyse fémorale. Aprèsajustement pour le poids, la masse maigre ou l’IMC, iln’y avait pas de différences entre les deux groupes concernantles indices de résistance osseuse (CSA, CSMI,Z, CT et BR) mesurés par le logiciel HSA au niveau ducol fémoral, de la région intertrochantérienne et de ladiaphyse fémorale.
